Please have all documents completed prior to arrival. Late arrivals or applicants who do not have their forms completed (in black ink only) prior to their appointment will be rescheduled for another day.            

 
REQUIREMENTS FOR OBTAINING A PASSPORT:

1. Appearance
  • Completed application in BLACK INK ONLY (type or print very clearly)
    Application forms are available for FREE from passport acceptance facilities or through www.travel.state.gov.
    DO NOT SIGN THE APPLICATION UNTIL ASKED TO DO SO

  • ALL applicants MUST appear in person, including minors.

  • Minors under age 16 must have BOTH parents present

  • Minors 16-17 must have at least one parent present.

2. Proof of U.S. Citizenship
  • Original/Certified copy of Birth Certificate (abstracts and Hospital Certificates are not acceptable)

  • Most recent issued U. S. Passport

  • Original Naturalization Certificate (only Certified Copies showing an agency seal are acceptable)

  • NOT ACCEPTABLE: Hospital Certificates, Baptismal Certificates, Notification of Birth Certificates, or photocopies of Certified Certificates

3. One Passport Photo
  • Photos must be 2" X 2" in size, clear and in color with a white or off-white background and represent a good likeness of the applicant.

  • Face the camera directly, no profile or ¾ profile photos. The full face must be visible and not covered by cast shadows, or clothing.
    NO FACE MASKS IN THE PHOTO

  • Neutral facial expression or natural smile, with both eyes open

  • No glasses

  • No uniforms, clothing that looks like a uniform, or camouflage attire.

  • No white shirts

  • No hats or head coverings (if you wear a hat or head covering for religious purposes, you must submit a signed statement that verifies that the hat or head covering in your photo is part of traditional religious attire worn continuously in public. If worn for medical purposes then you must submit a signed doctor's statement verifying that it is used daily for medical purposes).

  • No headphones or wireless hands-free devices

  • No "selfies"

  • Jewelry may be worn provided the face is not hidden.

 4. Valid Picture Identification
  • Current valid driver's license

  • California Identification Card

  • Military ID Card

  • Minors under age 16 will use the parent's I.D.; Minors age 16 - 17 must have own I. D.

5. Fees

Adult (age 16 and over):

$130.00 payable to the U. S. Department of State

Child (age 15 and under):
Valid for 5 years from the date of issue)

$100.00 payable to the U. S. Department of State

Passport Card (good for Land and Sea Travel, ONLY)

Adult (age 16 and over):
(Valid for 10 years from date of issue)

$30.00 payable to the U. S. Department of State

Child (age 15 and under):
Valid for 5 years from the date of issue)

$15.00 payable to the U. S. Department of State

Expedite Service Fee (Optional):

$60.00 payable to the U. S. Department of State

1 - 2 Day Delivery Fee for issued passport books
(Only applies to mailing addresses within the United States. Not valid for passport cards)

$21.36 payable to the U. S. Department of State

(Please note: Routine Service is currently approximately 6-8 weeks from submission of application to creation of passport. Expedited Service currently may take up to 2-3 weeks from submission of application to creation of passport. Mail times may vary.)

If you have your last issued passport in your possession, were over the age of 15 when your last passport was issued to you, and it has been less than 5 years since your passport book has expired then you may renew using form DS-82 and mail in your application yourself. All others including minors (ages 15 and younger) must reapply in person using form DS-11. See Renewal Form DS-82 for eligibility.
